IMPORTANT INFORMATION
This Re-enrolment Form serves two purposes:
  1. It confirms the selection of modules you will be taking for the following term.
  2. It indicates the mode of payment for individual modules either up front or via FEE-HELP loan.
Payment Information
If you are activating your FEE-HELP loan for the first time, you must complete a Request for FEE-HELP Assistance Form. There will be a link to this after you complete this form.

Please refer to the FEE-HELP information booklet to see if you are eligible for FEE-HELP.

Please be aware that FEE-HELP only covers the cost of tuition fees, you cannot use FEE-HELP to purchase textbooks and readings.

Students must return their completed Confirmation of Modules Form, including payment, on or before the Re-enrolment Date for the term. Forms and/or payment returned after this date will incur a late enrolment fee of $250.
If you are enrolling in Supervision or Practicum modules, it is a requirement that you are concurrently undertaking your placement. Your details must be registered with the Fieldwork Office.

Any further variations to the modules confirmed on this form must be submitted before the re-enrolment date for each term, or a late fee will be applied.
